GCCHD Vital Statistics

**As of October 2005 Birth and Death Certificates will be $19.00**

Eligibility: Birth and death records are public information and available to anyone upon request.

Purpose: To gather, compile and issue birth and death certificates.

Description: The Vital Statistics Office receives, records, and maintains records of all births and deaths occurring in Greene County from 1909 to present. All documents recorded prior to 1909 are located at the Greene County Records Center and Archives. Birth and death certificate information regarding the number of births, birth defects, and leading causes of death is maintained by the Ohio Department of Health. This information is used to assist in planning health programs to reduce handicapping conditions and to prolong life. Birth certificates are used for school registrations, social security numbers, passports, and retirement benefits. Death certificates are used for estate benefits and driver's license.

Fee:    Certified Birth and Death Certificates $19.00

Uncertified records are no longer available due to the passage of Ohio House Bill 95. We are sorry for any inconvenience that this may cause.

Cash only is accepted at the counter.  Money Orders and Certified Checks are accepted through the mail.  We do not accept any personal checks.

Please do not send cash through the mail. We are not responsible for lost mail.

Make Money Orders/Certified Checks payable to GCCHD. Please send business size (#10) self addressed, stamped envelope for certificates requested.

To speed up the process, you should call for your confirmation number to guarantee the certificate is on file at our office.
