The purpose of the Help Me Grow Contract for the Greene County Combined Health District is to provide clinical supervision and service coordination for Greene County families with Part C eligible at-risk children for referrals for specialized services in everyday routines and activities. It is our goal to create, nourish and maintain a
system of coordinated, community-based infrastructure that promotes
trans-disciplinary, family-centered services and supports for expectant
parents, newborns, infants, toddlers and their families in collaboration
with other state and local agencies. |
