Food Service Operations

1. Download, print, read and then complete the following items:
   A-Temporary Food License Brochure
   B-Temporary Food License Letter of Intent
   C-Application for a License to Conduct Temporary: FSO/RFE-
     
2. Contact our office to determine your license fees. Do not assume licensing fees, as they are dependent on types of food served and the type of organization serving them.
    
3. The Letter of Intent and Application must be received in our office at 360 Wilson Dr., Xenia, Ohio 45385, with the appropriate licensing fee, made out to the Greene County Combined Health District, a minimum of two business days prior to the event for a license to be issued. You may return applications by mail or in person Monday through Friday 8:00 AM - 4:00 PM.
