Birth Death Certificates
Birth and death records are public information and available to anyone upon request.
To gather, compile and issue birth and death certificates.
The Vital Statistics Office receives, records, and maintains records of all births and deaths occurring in Greene County from 1909 to present. All documents recorded prior to 1909 are located at the Greene County Records Center and Archives. Birth and death certificate information regarding the number of births, birth defects, and leading causes of death is maintained by the Ohio Department of Health. This information is used to assist in planning health programs to reduce handicapping conditions and to prolong life. Birth certificates are used for school registrations, social security numbers, passports,drivers license, and retirement benefits. Death certificates are used for estate benefits .

Certified Birth and Death Certificates $24.00
Cash and Credit/Debit cards are accepted at the counter. Money Orders and Certified Checks are accepted through the mail. We do not accept any personal checks.
You may order a certified copy of a birth, death or marriage record online using your credit card through our only approved Internet ordering service, VitalChek Network Inc.
The Greene County Combined Health District does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network Inc. An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard®, or Visa®.
Other internet vendors are not authorized by the Greene County Combined Health District and may charge additional fees without providing state-approved services. Neither the Greene County Combined Health District nor VitalChek have control over the services and fees other Internet vendors offer and charge, nor can we guarantee the confidentiality of any vital record data provided to vendors other than VitalChek
Make Money Orders/Certified Checks payable to GCCHD. Please send business size (#10) self addressed, stamped envelope for certificates requested.Please do not send cash through the mail. We are not responsible for lost mail.
