the H1N1 vaccine.
For seasonal flu, the cost
is $25.00 for adults and $15.00 for children.
We accept Medicaid, CareSource,

The GCCHD reminds parents and health care providers that children younger than 10 years of age should receive two doses of H1N1 flu vaccine in order to achieve optimal protection against pandemic flu. The two doses of H1N1 vaccine need to be at least 28 days apart to be effective.
Important for H1N1:
For faster
service at the clinic, please fill out the
H1N1 consent form. Download,
print and complete a form for each person who will be
receiving the H1N1 vaccination. Bring your completed form
with you to the clinic. To
schedule an appointment, please call (937) 374-5655.
