Important for H1N1:  For faster service at the clinic, please fill out the H1N1 consent form. Download, print and complete a form for each person who will be receiving the H1N1 vaccination. Bring your completed form with you to the clinic.

With any flu, prevention is very important.  Things you can do that will help to keep yourself and your family healthy include:

  • Wash your hands frequently, especially after using the bathroom, coughing or sneezing

  • Cover your mouth and nose when you cough or sneeze.  If you do not have a tissue, use your arm or sleeve.

  • Stay home if you are sick for at least 24 hours after you no longer have a fever, without taking fever-reducing drugs, such as Advil and Tylenol.

  • Avoid contact with people who are sick.

  • Get vaccinated!

There is an excellent question and answer section on the CDC website, http://www.cdc.gov/h1n1flu. You may also call the Ohio Department of Health H1N1 hotline at 1-866-800-1404.
